Abstract
The gynecologists who were trained in diagnostic laparoscopy have tried for several decades to utilize this trartsabdominal procedure for surgical intervention. With the development of several¢¥equipments including optical eildoscopes, powerful light sources, peritoneal insuff: lators, lasers and video monitoring.systems, the instruments also are developed and advanced tremendously. So that, many gynecologists are now doing more aggressive operative laparoscopic surgery to avoid unnecessary laparotomies for many patients.
In gynecologic field, variable laparoscopic surgery has been performed for many patients with problems such as tubal pregnancy, endometriosis, benign ovarian tumors or other adnexal tumors, uterine myoma, pelvic inflammatory disease and so on.From 1st August 1988 to 30th April 1991, 139 women with an pelvic tumor including uterine myoma were operated and followed up in our hospital by surgeons trained in pelviscopic surgery.
In our experiences, pelviscopic surgery has many advantages when compared with laparotomy as follows : shortening the operation time, less bleeding, reducing the duration of analgesic use, reducing hospital stay, reducing peritoneal adhesion, reducing out-of-work period, more tolerable although obese and possible early ambulation. . We concluded that the pelviscopic surgery should be the treatment for most benign pelvic masses when it was performed by the surgeon who had some experiences in pelviscopic surgery.
